1. Where there is a history of mental illness<\/strong><\/strong><\/h3><\/div><\/div>\n\n\n\n
\n\n\n\n

In women with Bipolar Affective Disorder, there is:<\/strong><\/strong><\/strong><\/p><\/div><\/div>\n\n\n\n

  • <\/i><\/span>

    More than one in five risk of suffering from postnatal psychosis.<\/p><\/span><\/span><\/span><\/li>

  • <\/i><\/span>

    Almost one in two risk of experiencing severe postnatal depression<\/p><\/span><\/span><\/span><\/li>

  • <\/i><\/span>

    One in two risk of postnatal psychosis for women with a history of previous postnatal psychosis<\/p><\/span><\/span><\/span><\/li><\/ul><\/div>\n\n\n\n

    \n\n\n\n

    These women should be referred to a community mental health team, preferably a specialist perinatal in pregnancy, even if they are well, so a personalised care plan can be made.<\/strong><\/strong><\/h3><\/div><\/div>\n\n\n\n
    \n\n\n\n

    In women with severe depression:<\/strong><\/strong><\/strong><\/strong><\/strong><\/p><\/div><\/div>\n\n\n\n

    • <\/i><\/span>

      There is a 40% risk of subsequent postnatal and non-postnatal relapse<\/p><\/span><\/span><\/span><\/li><\/ul><\/div>\n<\/div><\/section>\n\n\n\n

      2. Postpartum psychosis<\/strong><\/strong><\/strong><\/h3><\/div><\/div>\n\n\n\n
      \n\n\n\n
      • <\/i><\/span>

        It is a psychiatric emergency and requires specialist assessment and treatment within 4 hours<\/p><\/span><\/span><\/span><\/li>

      • <\/i><\/span>

        50% of women who get postnatal psychosis have no history of mental health problems<\/p><\/span><\/span><\/span><\/li><\/ul><\/div>\n<\/div><\/section>\n\n\n\n

        3.<\/strong> Presentations which should prompt urgent senior psychiatric assessment<\/strong><\/strong><\/h3><\/div><\/div>\n\n\n\n

          Recent significant change in mental state or emergence of new symptoms<\/p><\/span><\/span><\/span><\/li>

        • <\/i><\/span>

          New thoughts or acts of violent self-harm<\/p><\/span><\/span><\/span><\/li>

        • <\/i><\/span>

          New and persistent expressions of incompetency as a mother or estrangement from the infant<\/p><\/span><\/span><\/span><\/li><\/ul><\/div>\n<\/div><\/section>\n\n\n\n
