It is essential that healthcare professionals discuss the risks and benefits of use of medications in pregnancy and during breastfeeding to enable the woman to take an informed decision regarding their treatment. <\/p><\/div><\/div>\n\n\n\n

General principles of prescribing in the perinatal period:<\/strong><\/h3><\/div><\/div>\n\n\n\n
\n\n\n\n
  • <\/i><\/span>

    It should not be assumed that it is always better to avoid psychotropic medications<\/p><\/span><\/span><\/span><\/li>

  • <\/i><\/span>

    Use the lowest effective dose<\/p><\/span><\/span><\/span><\/li>

  • <\/i><\/span>

    Use the medication which is effective for women and has the lowest known risk to mother and foetus<\/p><\/span><\/span><\/span><\/li>

  • <\/i><\/span>

    Prescribe the least number of medications as possible<\/p><\/span><\/span><\/span><\/li>

  • <\/i><\/span>

    Document all decisions<\/p><\/span><\/span><\/span><\/li>

  • <\/i><\/span>

    Ensure that the mother and partner or family are as involved as possible in all decisions<\/p><\/span><\/span><\/span><\/li><\/ul><\/div>\n<\/div><\/section>\n<\/div><\/div>\n\n\n\n

      Woman\u2019s level of distress from untreated symptoms<\/p><\/span><\/span><\/span><\/li>

    • <\/i><\/span>

      Severity of previous episodes, previous response to treatment and the woman\u2019s preference<\/p><\/span><\/span><\/span><\/li>

    • <\/i><\/span>

      Potential effects of an untreated mental disorder on the foetus or infant (and the need for prompt treatment)<\/p><\/span><\/span><\/span><\/li>

    • <\/i><\/span>

      Risks of relapse or discontinuation symptoms from stopping the medication abruptly<\/p><\/span><\/span><\/span><\/li>

    • <\/i><\/span>

      Background risk of foetal abnormalities for pregnant women without a mental disorder<\/p><\/span><\/span><\/span><\/li>

    • <\/i><\/span>

      Uncertainty with regards to possible increased risk of harm associated with medications during pregnancy and the postnatal period; include the risk in overdose<\/p><\/span><\/span><\/span><\/li>

    • <\/i><\/span>

      The possibility that stopping a medication with known risk during pregnancy might not remove the associated risk<\/p><\/span><\/span><\/span><\/li>

    • <\/i><\/span>

      Where possible, written material (preferably individualised) should be provided to explain the risks<\/p><\/span><\/span><\/span><\/li><\/ul><\/div>\n<\/div><\/section>\n<\/div><\/section>\n<\/div><\/section>\n\n\n\n
